  • Manage a panel of approximately 6–8 patients per day

  • Assess and manage acute changes, chronic conditions, and post-hospital transitions

  • Lead daily rounding, including review and triage of overnight events with nursing staff

  • Support safe and efficient discharges back to primary care providers

  • Collaborate closely with physicians, nurses, rehab therapists, and care managers

Schedule

  • 0.6 FTE (30 hours/week)

  • Three 10-hour shifts per week (typically 7:00am – 6:00pm)

  • Rotating weekend phone call (7:00 AM – 7:00 PM, from home); paid call coverage

What You Can Expect Here

  • A collaborative, interdisciplinary care model across a 150+ bed facility

  • A highly team-oriented environment with strong nursing and leadership support

  • Meaningful continuity of care

Qualifications

  • NP or PA with (or eligible for) Vermont licensure

  • BLS certification required

Preferred:

  • 2+ years of experience in primary care, hospital medicine, urgent care, or post-acute care

  • Comfort managing medically complex adult or geriatric populations

  • Ability to work independently while collaborating closely with a multidisciplinary team

  • Given the complexity of the patient population, candidates with prior APP clinical experience are strongly preferred
