Job Description

The University of Vermont Medical Center is seeking an experienced Nurse Practitioner to join our Family Medicine team in Colchester, Vermont.

At Colchester Family Medicine, relationships matter. Join an established, collaborative team providing comprehensive primary care to patients and families across the lifespan.

Schedule

  • 0.75 FTE (30 hours/week) 

  • 24 hours of direct patient care with dedicated administrative time

  • Daytime schedule, Monday through Friday

  • Dedicated administrative time, with remote work opportunities available based on department needs

  • Quarterly Saturday weekend clinic participation following orientation

What You'll Do

  • Provide comprehensive outpatient primary care for patients across the lifespan

  • Manage and grow your own patient panel within an established Family Medicine practice with strong physician and APP collaboration

  • Diagnose and treat acute and chronic medical conditions

  • Deliver preventive care and wellness services

  • Conduct in-person and telemedicine visits

  • Collaborate with an interdisciplinary care team to support exceptional patient outcomes

Experience

  • Minimum of two years of Nurse Practitioner experience in Primary Care, Family Medicine, or a related outpatient setting required

  • Experience providing care across the lifespan required

  • Commitment to patient-centered, team-based care in an academic medical center 

Education & Licensure Requirements

  • Completion of an accredited Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) program

  • Current National Board Certification as a Nurse Practitioner

  • Active Vermont APRN license or eligibility for Vermont licensure

  • Eligibility for NY licensure 

  • Eligibility for prescriptive authority

  • Eligibility for DEA licensure 

Telemedicine opportunities are available. Providers must be eligible to obtain both Vermont and New York licensure to support virtual care for patients located in New York.
